Job summary

Hannibal Regional Healthcare System, Inc. seeks a dedicated Case Manager for Rehabilitation to coordinate care in an inpatient rehabilitation setting.

You will work a 40-hour week, Monday through Friday, collaborating with nursing, therapy and other departments to ensure effective resource use and patient outcomes. Join a fast-paced, team-oriented unit focused on quality care, discharge planning, and payer communications.

Qualifications

• Certification in Case Management or Rehabilitation Nursing is preferred (CCM, ARN, ACM or ANCC CM).
• Minimum of 2 years social work or case management experience in inpatient settings preferred; acute/rehab hospital experience valued.
• Strong oral and written communication in English; additional languages preferred.
• Basic computer skills: Excel, Word, Outlook, PowerPoint.

Responsibilities

• Coordinates management of care to ensure optimal utilization of resources and services.
• Performs comprehensive assessments, care planning, implementation and evaluation for patient needs.
• Completes IPoC per CMS guidelines and documents payer communications in Meditech.
• Schedules family conferences and communicates progress to patients and caregivers.
• Leads weekly patient care team conferences to refine treatment plans and outcomes.
• Coordinates discharge planning including home health, equipment, and follow-ups.
