Job summary

Lifepoint Rehabilitation seeks a dedicated Case Manager for Hannibal Regional Hospital’s rehabilitation program. This full-time, dayshift role coordinates care, ensures efficient resource use, and supports discharge planning to promote optimal patient outcomes.

The role emphasizes comprehensive assessment, care planning, and collaboration with nursing, therapy, and ancillary teams to maintain high-quality patient care. Experience in inpatient settings is preferred.

Qualifications

• 2+ years social work or case management in an inpatient setting preferred.
• Certification in Case Management or Rehabilitation Nursing preferred (CCM/ARN/ACM/RN-BC).
• Excellent oral and written communication skills in English.
• Good organizational skills and ability to manage time and workflow.
• Must be a cooperative team player with confidentiality awareness.

Responsibilities

• Coordinate management of care, optimize resource use and discharge planning.
• Perform comprehensive assessments, care planning and plan implementation.
• Lead IPoC development per CMS guidelines and document care in Meditech.
• Schedule family conferences and communicate progress to patients and caregivers.
• Contribute to performance improvement initiatives and cross-department collaboration.
