Job summary

Lifepoint Health is seeking a dedicated Case Manager for the Rehabilitation unit at Hannibal Regional Hospital in Hannibal, MO. This full-time, 40-hour-per-week role in a Days shift focuses on coordination of care, discharge planning, and collaboration with therapy and nursing teams to optimize patient outcomes.

You will perform assessments, care planning, and implementation while ensuring CMS guidelines and payer communications are managed effectively.

Qualifications

• Certification in Case Management or Rehabilitation Nursing preferred; CCM/ARN/ACM or CM certs recognized.
• Minimum of 2 years social work or case management experience in an inpatient setting preferred.
• Effective oral and written communication in English; bilingual skills a plus.
• Basic computer skills: Excel, Word, Outlook, PowerPoint; data entry and documentation.
• Strong organizational skills, time management, analytical ability; independent work and secrecy.

Responsibilities

• Coordinates management of care and ensures optimum use of resources and compliance with review agencies.
• Performs comprehensive assessments, care planning, implementation, and evaluation of patient needs.
• Schedules family conferences and communicates progress and discharge plans to caregivers.
• Leads weekly care team conferences to monitor treatment plans and patient goals.
• Ensures CMS discharge planning compliance and coordinates post‑acute services as needed.
• Performs intake assessments within 24–72 hours of admission and follows up per policy.
• Collaborates with nursing, therapy, and ancillary staff to ensure proper utilization.
• Participates in utilization review, denials, and payer communications as required.
