Job summary

Tidelands Health Rehabilitation Hospital, an affiliate of Encompass Health, is seeking a Case Manager to coordinate patient care from admission to discharge and guide interdisciplinary plan-of-care decisions.

This role emphasizes home/community impact and requires licensure and CCM/ACM certification within two years; it includes comprehensive health plans and opportunities for professional growth.

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• Active license required if licensure is needed for one's discipline within the state.
• CCM® or ACM™ certification required or must be obtained within two years of entry into the position.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Facilitate weekly team conferences and coordinate all treatment plan modifications.
• Complete case management addendums and all required documentation.
• Maintain knowledge of regulations, policies, procedures, and department operations.
• Review case management reports and plan appropriate actions.
• Attend ACT meetings to identify trends and reduce ACTs.
• Meet with patient/family within 24 hours of admission.
• Assess goals and complete case management addendum within 48 hours of admission.
• Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
• Schedule and facilitate family conferences as needed.
• Assist patient with resources to avoid discharge delays.
• Monitor compliance with orthotics and prosthetics ordering and payment.
• Make referrals and document to post-discharge providers.
