Job summary

Advocate Health is seeking an experienced Registered Nurse to coordinate rehabilitation care and advocate for patients within the care team. You will plan, organize, and monitor services to address each patient’s health care needs, while ensuring quality outcomes.

You will evaluate progress, apply holistic assessments, and identify functional changes. BSN is required; CRRN preferred and/or obtained. BLS certification is also required.

Qualifications

• Bachelor's Degree in Nursing required; Master's in Nursing preferred.
• Licensed to practice as RN in the applicable state; CRRN preferred and/or obtained.
• 3 to 5 years of related clinical experience in rehabilitation of chronically or catastrophically ill or injured individuals is highly recommended.
• If employee already has a BSN, they have 2 years to become CRRN certified.
• BLS required per policy guidelines.

Responsibilities

• Plans, organizes, coordinates, and monitors services and resources needed to respond to an individual's health care needs.
• Provides ongoing evaluation of the individual's progress on the plan and effectiveness of the services throughout hospitalization.
• Represents the patient in the care team and advocates for effective services to ensure quality of care and attainment of goals.
• Integrates knowledge of disease and medical treatments in a holistic assessment, focusing on differential diagnosis.
• Identifies any temporary or permanent alterations in function and formulates nursing diagnosis.
• Identifies potential challenges or complications in physiological and/or psychosocial function.
• Assists the individual and family in anticipating needs and planning for re-entry to home or alternative living site.
• Designs, implements, and evaluates population-based programs of care by integrating nursing interventions and medical treatments.
• Assesses, plans, implements and evaluates formal and informal educational opportunities and materials for nursing staff and patients/families.
• Leads multidisciplinary groups in designing and implementing innovative alternative solutions across the full continuum of care.
