Advocate Health is seeking an experienced Registered Nurse to manage an assigned patient caseload in home health, ensuring timely care and accurate documentation according to the established plan of care and physician orders.

The role involves data collection from patient assessments and EMR, analyzing risks, coordinating a multidisciplinary team, and supervising LPNs and HHAs to promote high-quality outcomes and patient satisfaction in the home setting.

Qualifications

• Graduate from an accredited School of Nursing; RN license required.
• BSN preferred; BLS required.
• Experience in home health care preferred.
• Ability to supervise LPNs and HHAs.

Responsibilities

• Manage daily patient caseload and ensure timely care and documentation per plan of care.
• Collect data from home visits, telephonic contacts and EMR.
• Analyze data to identify needs and coordinate care to prevent high utilization.
• Prioritize issues based on patient health and safety considerations.
• Coordinate multidisciplinary clinical team and supervise LPNs and HHAs.
• Perform required treatments and procedures per regulatory standards.
• Evaluate progress toward patient-centered goals.
