Responsibilities

• Monitor and manage clinical and financial coordination of treatment plans.
• Ensure patient progression through the continuum of care while achieving clinical and financial outcomes.
• Coordinate length of stay within CMS guidelines and compliance thresholds.
• Connect patients with community resources including vocational rehabilitation when appropriate.
