Job summary

You will monitor clinical and financial aspects of treatment plans for patients with spinal cord injury, brain injury, stroke, amputations, and other conditions, aiming for timely, cost-effective, individualized service delivery.

Qualifications

• Current state licensure as a Registered Nurse or Licensed Social Worker.
• MSW without licensure will be considered.
• Minimum of 3 years healthcare and clinical experience in a hospital setting.

Responsibilities

• Collaborates with physicians to manage patient programs and ensure progression through the continuum of care.
• Monitors clinical and financial coordination of treatment plans for timely, cost‑effective service delivery.
• Coordinates length of stay within CMS guidelines and provides community resources when appropriate.
