• 1 year hospital experience preferred.
• Certification in case management preferred.

Responsibilities

• Provide individual program management for each patient and ensure progression through care continuum.
• Monitor and coordinate clinical and financial aspects of treatment plans.
• Coordinate length of stay within CMS guidelines and 60% compliance threshold.
• Provide community resources including vocational rehabilitation when applicable.
