Job summary

Encompass Health Corporation in Florida is seeking a Case Manager to coordinate patient care from admission to post-discharge, collaborating with an interdisciplinary team to optimize outcomes. This role emphasizes discharge planning, care coordination, and timely documentation, with a focus on delivering compassionate rehabilitation services close to home.

The Case Manager will ensure regulatory compliance and contribute to a patient-centered discharge experience at a nationwide inpatient

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of one's discipline (e.g., RN, SW, OT, PT, ST, Rehabilitation Counseling).
• Active license if licensure is required for the discipline within the state.
• CCM® or ACM™ certification required OR must be obtained within two years of entry into the position.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
• Facilitate team conferences weekly and coordinate all treatment plan modifications.
• Complete case management addendums and required documentation.
• Maintain knowledge of regulations/policies and department operations.
• Review/analyze case management reports and plan appropriate actions.
• Understand commercial contracts, exclusions, payor requirements, and recertification needs.
• Attend ACT meetings to identify trends and collaboratively reduce ACTs.
• Meet with patient/family within 24 hrs of admission.
• Assess goals and complete case management addendum within 48 hours of admission.
• Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
• Schedule and facilitate family conferences as needed.
• Assist patient with timely procuring/planning of resources to avoid discharge delays.
• Monitor compliance with orthotics and prosthetics ordering and payment regulations.
• Make referrals to post-discharge providers/physicians.
• Ensure accuracy of discharge and payor information in patient records.
• Participate in utilization review: data collection, trend review, resolution actions.
• Participate in case management on-call schedule.
