Job summary

PeaceHealth Springfield Rehabilitation Hospital in Springfield, OR, is opening a brand-new inpatient rehabilitation facility. We are seeking a PRN Case Manager to join a mission-driven team delivering patient-centered care and coordination across admissions, treatment planning, and education.

The role emphasizes pre-admission screening, initial assessments, treatment planning, and collaboration with patients, families, and insurance providers to obtain pre-certification and support ongoing care.

Qualifications

• Must hold OR licensure for assigned profession (RN/LCSW/PT/SLP).
• Inpatient case management or rehabilitation experience preferred.
• Excellent communication in English; multilingual a plus.
• Proficient with standard office software.
• Ability to maintain confidentiality and work independently.

Responsibilities

• Participate in pre-admission screening as requested.
• Conduct initial assessments and develop individual treatment plans.
• Provide age-appropriate therapeutic interventions.
• Evaluate patient and family response to treatment and document progress.
• Manage insurance pre-certification & re-certification as appropriate.
• Collaborate with team to support patient goals and education.
