The ideal candidate will conduct assessments, develop treatment plans, and work closely with patients and families to ensure continuity of care. Essential qualifications include current licensure in Oregon and a minimum of 2 years of experience in social work or case management. Qualifications

• Minimum of 2 years social work or case management experience in an inpatient setting.
• Must recognize and observe confidentiality in all interactions.
• Analytical ability to interpret information and carry out duties independently.

Responsibilities

• Participate in pre-admission screening as requested.
• Conduct initial assessments and develop individualized treatment plans.
• Evaluate patient and family response to treatment and document care.
• Establish and maintain therapeutic relationships with patients and families.

Skills

Basic computer skills

Education

Current RN, LCSW, RT, PT, SLP licensure in the state of OR

Certification in Case Management or Rehabilitation Nursing preferred
