Job summary

PeaceHealth Springfield Rehabilitation Hospital, part of Lifepoint Health, is opening a new inpatient rehabilitation facility. We seek a Case Manager with OR licensure in RN/LCSW/RT/PT/SLP to join our team on a full-time basis.

Responsibilities include pre-admission screening, assessments, treatment plans, and coordinating insurance pre-certifications. Benefits, continuing education support, and a $10,000 sign-on bonus are offered.

Qualifications

• Current RN, LCSW, RT, PT, or SLP licensure in OR.
• Certification in Case Management or Rehabilitation Nursing preferred.
• Minimum of 2 years social work or case management in an inpatient setting.

Responsibilities

• Participate in pre-admission screening as requested.
• Conduct initial assessments and develop individualized treatment plans.
• Provide age-appropriate therapeutic interventions and education.
• Obtain pre-certification/re-certification of insurance coverage.
• Maintain patient records and communicate progress to families.
