Florida Rehabilitation Hospital at Tampa is seeking a Case Manager to coordinate patient care alongside physicians in a 60-bed rehab setting. You will monitor treatment plans, ensure timely, cost-effective service delivery, and facilitate progression through the continuum of care for patients with spinal cord injury, brain injury, strokes and orthopedic conditions.

Requirements include active RN or MSW licensure, at least 3 years of healthcare hospital experience; MSW without licensure

Competenze

• Current state licensure as RN or MSW.
• MSW without licensure will be considered.
• Minimum of 3 years healthcare and clinical experience in a hospital setting.

Mansioni

• Coordinate patient care in collaboration with physicians to ensure progression through care continuum.
• Monitor and manage clinical and financial coordination of treatment plans for assigned patients.
• Assist patients with various rehabilitation needs and coordinate CMS guideline compliance.
