Responsibilities

• Coordinate health care decisions using a systematic approach to improve treatment plans and discharge planning.
• Coordinate care across the continuum and promote cost-effective hospital-based services.
• Assess psychosocial needs of the patient and provide discharge planning interventions.
• The primary job functions include Clinical Interventions/Discharge Planning, Care Planning Management, Fiscal Management, and Payer/Referral Management.
