Job summary

Spaulding Rehabilitation Cape Cod, part of Mass General Brigham, seeks a part-time per-diem care coordinator to join its Cape Cod team. The role coordinates care, supports referrals, and helps with discharge planning across inpatient/outpatient programs.

Qualifications

• RN license (Massachusetts) or PT/OT/SLP license as applicable.
• BLS would be preferred.
• 2-3 years of case management, discharge planning, or utilization review experience preferred.

Responsibilities

• Coordinate and support care within the facility and referrals for services outside the clinic.
• Develop and coordinate community service plans with families and providers.
• Prepare patients and families for discharge and obtain necessary authorizations for reimbursement.
