Job summary

Spaulding Rehabilitation Hospital Corporation in Cambridge is seeking a skilled case manager to support LTAC patients. This full-time, day-shift role focuses on intake interviews, evaluating medical and psychosocial needs, and coordinating services with the clinical team.

The ideal candidate holds a Bachelor's degree in a related field, an MA license, and 2–3 years of discharge planning or utilization review experience.

Qualifications

• Bachelor's degree in related field required.
• Licenses: RN, PT, OT, SLP, RT or LICSW/LCSW per role.
• 2–3 years in case management, utilization review, or discharge planning preferred.

Responsibilities

• Complete intake interviews and determine need for medical and psycho-social evaluations.
• Review therapist and physician evaluations, care objectives and plans; coordinate services.
• Advocate for needed services and patient entitlements.
• Discuss assessment findings with physician/treatment team to maximize outcomes.
• Coordinate discharge and aftercare with guidance from the clinical team.
• Ensure compliance with state and federal regulations and credentialing bodies.
