Qualifications

Responsibilities

• Assist case managers and social workers with referrals to SNF, home care, and outpatient; manage communications with facilities and start of care dates.
• Support the insurance authorization and continued stay process by contacting the insurance CM and submitting requests.
• Research available community resources per CM/SW request; assist patients and families with applications for resources.
• Maintain a database for insurance-based processes, including denials and appeals, Medicaid conversions.
