Job summary

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice
• Active license required in the state
• CCM or ACM certification required or to be obtained within two years
• Strong knowledge of discharge planning and patient advocacy

Responsibilities

• Coordinate care from admission to post-discharge within an interdisciplinary team
• Establish and monitor discharge plans and contingency plans
• Participate in utilization review and case management reporting
• Educate patient/family on rehabilitation and care plan
• Ensure accuracy of documentation and compliance with regulations
