Job summary

NeuLife Rehabilitation of Michigan is seeking a Case Manager to coordinate care for individuals with traumatic brain injuries and spinal cord injuries. You will work with an interdisciplinary team, clients, families, guardians, payers, and external case managers to create individualized plans and monitor progress.

Requirements include at least three years in case management in rehab or healthcare, with a preferred Bachelor’s degree and CCM/ACM certification.

Qualifications

• At least three years of case management experience, preferably in rehabilitation or related healthcare.
• Bachelor's degree in a related field preferred.
• CCM or ACM certification preferred.

Responsibilities

• Assess needs and coordinate individualized care plans with the interdisciplinary team
• Monitor progress and communicate updates to clients, families, guardians, and providers
• Coordinate care conferences, utilization reviews, and payer authorizations
• Support discharge planning, referrals, and transitions of care
• Maintain accurate documentation and track quality measures
