Qualifications

• Minimum of two years of satisfactory employment experience in direct case management or related field.
• Equivalent training and experience may be considered.

Responsabilités

• Manages and monitors assigned caseload of Beneficiaries in a rehabilitation program.
• Develops program plan/goals and evaluates beneficiaries' progress by conducting routine meetings with Beneficiaries and staff.
• Serves as advocate for Beneficiaries to acquire services to enable functioning in their environment.
