Job summary

St. John’s Regional Medical Center is seeking a Rehab Liaison to coordinate the acute rehabilitation admission process, including pre-admit screening and financial authorization. You will partners with the program medical director and rehab director to ensure seamless patient flow.

You will actively nurture existing referral sources while also developing new relationships, consulting with physicians, and ensuring comprehensive pre-screening documentation for admissions.

Qualifications

• Minimum of five (5) years clinical experience.
• Licensure: RN CA or CA-PT / CA-OT / CA-SLP.
• Bachelor's Degree required.

Responsibilities

• Coordinate acute rehabilitation admission process including pre-admit screening and financial authorization.
• Communicate with program medical director and rehab director daily.
• Engage referral sources and cultivate new clients for expanded referrals.
• Consult with physicians to complete pre-screening documentation and collect necessary information.
