Job summary

CHRISTUS Health in Santa Fe, NM seeks a Rehab Liaison to evaluate candidates, facilitate admissions to the Inpatient Rehabilitation Unit, and coordinate with admissions, social work, and payers.

You will conduct pre-admission assessments, verify insurance, obtain authorizations, and serve as a liaison with providers and community partners to improve rehab outcomes and transitions. This role is PRN and Per Diem as needed.

Qualifications

• Current license in New Mexico as Clinician (Nursing, Occupational Therapist, Physical Therapist, LISW, or SLP).
• Five years of clinical experience, with at least two years in acute rehabilitation.
• Experience in management or a managed care role is preferred.

Responsibilities

• Conducts pre-admission assessments for Rehabilitation services.
• Early identification of candidates and potential clients.
• Reviews referral information with Rehabilitation team and Medical Director to decide level of care.
• Verify eligibility with insurance carriers and secure authorizations prior to admission.
• Facilitate benefit eligibility and precertification with payers.
• Coordinate admission process and transitions across care settings.
• Maintain outreach and data/outcomes analysis for rehabilitation services.
• Liaise with physicians, nurses, and social workers to promote referrals and smooth transitions.
