Job summary

Exalt Health Rehabilitation Hospital Lakeland LLC seeks a Clinical Liaison to serve as the primary contact between the inpatient rehab hospital and referral sources. You will educate the community on rehab services, identify suitable patients for transfer, and facilitate smooth admissions.

The role requires building relationships with hospitals, nursing facilities, physicians, and case managers, plus conducting on-site assessments and ensuring timely admissions.

Qualifications

• Active healthcare license preferred in nursing, therapy or SLP.
• Valid driver’s license and clean driving record.
• Minimum 2–3 years of clinical experience in acute care or rehabilitation.

Responsibilities

• Establish and maintain relationships with case managers, physicians, therapists, social workers and other professionals.
• Conduct regular visits to promote hospital services and awareness.
• Perform pre-admission assessments at referral sources to determine suitability.
• Review medical records and educate patients and families about rehabilitation benefits.
• Coordinate transfer and admission processes including insurance verification.
• Develop and distribute marketing materials with the Business Development team.
• Travel daily within a designated territory to visit referral sources.
• Track referrals, admissions, and other metrics; report to Director of Business Development.
