Job summary

Diablo Valley Post Acute in Concord, CA is seeking an Occupational Therapist (OTR) to join our in-house rehab team. We offer full-time, part-time, and PRN options with competitive hourly pay and flexible scheduling to support work-life balance.

You will deliver comprehensive occupational therapy services, develop individualized plans, and collaborate with a multidisciplinary team to enhance residents' independence and quality of life.

Qualifications

• Current California Occupational Therapist (OTR) license in good standing.
• Strong communication and interpersonal skills.

Responsibilities

• Perform comprehensive occupational therapy evaluations.
• Develop and implement individualized treatment plans.
• Provide evidence-based interventions focused on improving functional independence and quality of life.
• Address activities of daily living, mobility, coordination, strength, cognition, and other functional needs.
• Monitor resident progress and modify treatment plans as appropriate.
• Educate residents, families, and caregivers on therapeutic strategies and safety.
• Collaborate with the interdisciplinary team to coordinate care and discharge planning.
• Maintain accurate, timely, and compliant clinical documentation.
