Job summary

Ascension is seeking a compassionate Registered Nurse for the All Saints Hospital in Racine, WI. The role is day shift, full-time within the Vascular Interventional Pre/Post Procedural department.

Provide direct nursing care, perform physician orders, monitor patient status, and coordinate with multiple disciplines to ensure continuity of care and a safe discharge plan. Requires BLS within 1 month, RN licensure in WI (or multi-state/Compact) or MN, and a nursing diploma or equivalent licensure

Qualifications

• BLS Provider within 1 Month (30 days) of hire date or job transfer date required.
• RN credentialed from the Wisconsin Board of Nursing with WI licensure prior to hire, or current home state license if multi-state/Compact State.
• RN credentialed from the Minnesota Board of Nursing prior to hire, licensure dependent on state of practice.

Responsibilities

• Deliver compassionate, high-quality direct nursing care in accordance with established protocols to promote patient safety and optimal clinical outcomes.
• Execute physician and nursing orders promptly, meticulously implementing and monitoring individualized care plans to advance patient recovery.
• Monitor and communicate critical patient status updates to the multidisciplinary care team to ensure timely interventions and seamless continuity of care.
• Coordinate services across all clinical disciplines, serving as the primary liaison to guarantee a well-integrated, patient-centered care journey.
• Facilitate comprehensive discharge planning by evaluating patient needs and collaborating with the care team to ensure a safe, smooth transition home.

Licensure/Certification/Registration

What minimum requirements you’ll need

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• One or more of the following required:
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.
• Registered Nurse credentialed from the Minnesota Board of Nursing obtained prior to hire date or job transfer date. Licensure is dependent on state associate works in.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
