Job summary

Outcome HC in the Philadelphia area is seeking a Registered Nurse for a unit-based role. The RN is responsible for assessment, medication distribution, clinical documentation, and supervising CNAs and other staff, under the supervision of the DON/ADON.

The position emphasizes teamwork with multidisciplinary teams, adherence to infection control, SBAR communication, and timely nursing assessments on admission and discharge.

Qualifications

• Graduate of an accredited school of nursing and licensed by the PA as an RN.
• Current CPR or certified within 90 days of hire.
• Other certifications may be required for other areas of the organization.

Responsibilities

• Assumes responsibility for a unit including assessment under supervision.
• Medication distribution, treatment documentation, and supervision of CNAs.
• Uses SBAR for communication with healthcare team.
• Transcribes physician orders on MAR and TAR.
• Maintains safety, infection prevention, and adherence to policies.
• Admits and discharges residents per facility procedure.
