Job summary

Outcome HC in Philadelphia is seeking a Unit Manager to oversee assessment, medication distribution, clinical documentation, and supervision of CNAs and staff in the designated unit.

Under the supervision of the DON/ADON, you will lead care plans, ensure infection control, and coordinate with multidisciplinary teams to maintain resident care standards. This is a full-time on-site role in Pennsylvania with benefits and competitive pay.

Qualifications

• Graduated of an accredited school of nursing and licensed by the state of PA as a LPN/RN.
• Current CPR or certified within 90 days of hire.
• Other certifications may be required for other areas of the organization.

Responsibilities

• Uses dual identifiers to confirm resident/guest identity prior to medication administration and treatments.
• Practices SBAR (situation, background, assessment, recommendation) in communication with the healthcare team.
• Encourages resident/guest involvement in their own care when possible.
• Receives full and detailed report from outgoing nurses for proper communication and follow-up.
• Recognizes and responds to changes in resident condition and documents appropriately.
• Initiates and updates Plan of Care on admission and through care.
• Interacts with multidisciplinary teams to assure continuum of care.
• Practices Standard Precautions and infection prevention strategies.
• Demonstrates compliance with Resident’s Rights.
• Follows established policies for medication administration, MAR/TAR transcription, and orders.
• Maintains a safe environment and follows chain of command.
• Admits and discharges residents/guests per facility procedures.
• Reviews reports with CNAs prior to assignment activation.
• Performs assessments across multiple systems (skin, cardiac, respiratory, GI, neurological, MSK, renal, pain).
• Completes nursing assessments and data collection on admission (Fall Risk, Braden Scale, immunizations, etc.).
• Reconciles medications and documents legibly onto POS; transcribes and verifies telephone orders.
• Faxes POS to Pharmacy and makes referrals as needed.
