Job summary

UPMC Northwest is seeking a flexible full-time Registered Nurse to join the Transitional Care Unit team. This role covers a variety of shifts on the Transitional Care and Rehab Units, with a career ladder that reflects education, experience and manager discretion at offer.

The RN will partner with patients and families, develop care plans, teach and mentor, and participate in quality improvement to ensure safe, effective post-acute care across units. BSN preferred and 0–2 years' experience.

Qualifications

• BSN preferred and RN licensure in the U.S.
• Zero to two years of nursing experience; good clinical judgment.
• Ability to work across Transitional Care and Rehab Units with flexibility.

Responsibilities

• Develops and maintains productive working relationships and communicates safety hazards.
• Teaches patients and families, may serve as a preceptor and guide transitions of care.
• Applies nursing process to create a holistic, patient-centered plan of care.
• Participates in quality improvement and supports change to improve patient outcomes.
• Demonstrates accountability for professional development and teamwork.
