Job summary

UPMC Northwest is hiring a flexible full-time Registered Nurse to join the Transitional Care Unit team. This role will work a variety of shifts on both the Transitional Care and Rehab Units, with candidates placed on the career ladder reflecting education, experience, and manager discretion at offer.

The RN will develop productive working relationships, teach patients and families, apply the nursing process, and support continuous improvement through governance and quality initiatives.

Responsibilities

• Develops and maintains productive working relationships internally and externally by demonstrating accountability for actions, enthusiasm, motivation and commitment to patients and colleagues.
• Demonstrates knowledge of adult learning principles and applies in teaching of patients, families, students and new staff; provides detailed teaching to guide through the episode of care and transition to home or another level of care.
• Applies the nursing process within Relationship Based Care to create a healing environment; formulates daily goals and a plan of care with patient partnership, considering holistic needs.
• Demonstrates accountability for professional development, participates in unit-based governance, and supports change to improve quality of care and practice environment.
• Participates in department/unit quality improvement efforts; identifies opportunities for quality improvement and contributes to patient satisfaction and evidence-based practice.
