Job summary

UPMC Northwest is looking for a flexible full-time Licensed Practical Nurse (LPN) to join the Transitional Care Unit, with shifts on the Transitional Care and Rehab Units and a career ladder reflecting education, experience, and manager discretion at offer time.

The LPN collaborates with the health-care team, uses sound nursing judgment, follows UPMC guidelines and state regulations, and provides patient education and ongoing professional growth within a compassionate, healing environment.

Responsibilities

• The LPN participates in planning, implementation and evaluation of nursing care.
• Communicate with the licensed professional nurse and health care team to seek guidance when patient care needs exceed the LPN scope.
• Follow UPMC policies and LPN Practice Guidelines and the State Practice Act.
• Create a healing environment with a patient-centered, holistic plan of care and daily goals.
• Provide teaching to patients and families and encourage ongoing professional development.
• Develop and maintain productive relationships and communicate safety concerns to improve patient care.
