Job summary

UPMC Northwest is hiring a flexible full-time LPN to join the Transitional Care Unit team. This role will work a variety of shifts on both the Transitional Care and Rehab Units.

Final candidates will be selected for a job title within the career ladder that reflects level of education, experience, and manager discretion at time of offer. Responsibilities include nursing judgment, care planning, and collaboration with the health care team while ensuring patient safety and quality care.

Qualifications

• Graduate from a Licensed Practical Nursing Program approved by the State Board of Nursing.
• Ability to provide care and interact appropriately with patients across all ages served.
• Experience in health care preferred, no LPN experience required.
• Ability to communicate with executives, managers, physicians, and care team members.

Responsibilities

• Function as a member of the health-care team and use nursing judgment based on education and experience.
• Plan, implement and evaluate nursing care using focused assessment.
• Communicate with licensed professionals when care needs exceed LPN scope or knowledge.
• Provide patient and family teaching to guide through care and transition.
• Build healing relationships and actively participate in unit activities.
