Job summary

Solara Care Solutions is seeking a PRN Registered Nurse to deliver exceptional clinical care and leadership in home health settings. You will perform direct nursing services, case management, and ensure regulatory compliance while maintaining the highest standards of client care.

The role emphasizes collaboration with interdisciplinary teams, timely assessments, and education for clients and families. Flexible PRN hours with competitive compensation and benefits are offered.

Qualifications

• Must be a licensed Registered Nurse with Florida license.
• Current CPR certification required.
• Must have valid state driver’s license and proof of motor vehicle insurance.

Responsibilities

• Observation, assessment, nursing diagnosis, counseling, care and health teaching of the ill, injured or infirm.
• Complete initial assessment within 24–48 hours of client admission and prepare a care plan.
• Explain procedures, treatments, medications, and plan of care to client and family as appropriate.
• Provide progress notes to client’s healthcare provider and report changes of conditions or deviations from the plan at least every 60 days.
• Direct activities of clinical care to LPNs, CNAs and other staff.
• Provide written instructions and supervise staff in the home.

Key Responsibilities

• Complete initial assessment within 24‑48 hours of client admission and prepare a care plan.
• Explain all procedures, treatments, medications, and client’s plan of care to client and family as appropriate.
• Provide progress notes to client’s healthcare provider, report changes of conditions or deviations from the plan or care, or at least every 60 days.

• Provide written instructions and supervise staff in the home.

Qualifications

• Must be licensed Registered Nurse and hold a valid license for the State of Florida or show proof of an application for such.
• Current valid CPR certification required.
• Must have valid state driver’s license and proof of motor vehicle insurance.

Experience

Must have knowledge of all phases of nursing care to residents and previous experience in a convalescent home, skilled nursing facility, or general hospital with supervisory or charge nurse abilities.

Some experience in geriatrics is preferable.
