Fleet Landing and Solara Care Solutions invite you to join as a PRN Physical Therapist Assistant in Atlantic Beach, FL. This role supports independence, dignity, and quality of life for seniors through in-home and outpatient therapy services.

Qualifications

• Current license from Florida Department of Health.
• Completion of an accredited Physical Therapist Assistant program.
• Current CPR.

Responsabilités

• Assist with client screenings, including new admissions within 24 hours and long-term residents experiencing significant change or referred by nursing.
• Implement treatment programs for clients as directed by the Physical Therapist and supported by doctor's orders.
• Complete all required documentation accurately and timely, including daily treatment notes, weekly progress notes, monthly recertifications, and discharge summaries, co-signed by the supervising Physical Therapist.
• Schedule clients appropriately on the assigned caseload.
