Job summary

Meadow Park Rehabilitation & Health Care Center is seeking an RN Supervisor to manage our subacute unit, overseeing nursing care and coordinating with physicians and the interdisciplinary team.

The role requires NY RN licensure, prior supervision experience in a long-term care setting, and proficiency with EMR systems. Strong communication skills are essential to support patients, families, and staff.

Qualifications

• Must hold NY RN license.
• Previous experience as RN Supervisor in a long-term care facility.
• Familiarity with EMR systems and clinical protocols.

Responsibilities

• Managing the individualized resident care by promoting and monitoring residents' health through the nursing process.
• Utilizes clinical resources in implementing patient care and provides rationale for patient care decisions.
• Collaborating with physicians and interdisciplinary team members to resolve all residents' problems and needs
• Providing physical and psychological support to patients and their family members
• Monitoring residents' conditions and administering medication & treatments
• Supervising assigned team members.
• Completes Frequent Observations and the Intake/Outtake Record according to established protocols.
• Performs related duties, as required.
