We are seeking a PRN RN Care Coordinator responsible for coordinating patient care across acute, chronic, and long-term stages.

Key duties include

• utilization management, transitional care planning, psychosocial and functional assessments, patient advocacy, and education for patients and families
• Collaborating with healthcare team members to develop comprehensive care plans aimed at achieving optimal outcomes
• Implementing discharge planning, addressing barriers to timely disposition, and providing resource information
• Assessing and ensuring appropriate use of healthcare services using established criteria (e.g., Interqual), monitoring ongoing plans, and facilitating modifications as needed
• Managing insurance and coverage issues, supporting performance improvement initiatives, and ensuring compliance with regulations
• Completing accurate documentation, participating in evidence-based practice, and serving as a mentor or preceptor to colleagues and students.

Qualifications Include

Associates or Bachelor’s nursing degree, valid RN license, BLS certification, and minimum of 3 years clinical experience.
