Role Overview:

The Care Coordinator RN (CC RN) assesses and coordinates transitional care needs, ensuring seamless patient care across settings.

Responsibilities:

Conducts psychosocial and functional assessments, develops individualized discharge plans, facilitates patient and family education, and collaborates with interdisciplinary teams to promote efficient care progression. Manages discharge processes, identifies barriers, and initiates post-acute referrals, working closely with physicians, social workers, and other care professionals. Ensures accurate documentation of assessments, care plans, and interactions, tracking trends to improve outcomes.

Qualifications & Skills:

Requires an RN license (single or multi-state), BLS certification, and at least one year of acute care nursing experience. Candidates must demonstrate excellent communication, organizational, and problem-solving skills, with the ability to work under stress and in fast-paced environments. Knowledge of regulations and community resources is essential.
