Job summary

AvidaHealth is seeking a Registered Nurse (RN) on a PRN, as-needed basis to provide in-home personal care across Milwaukee and Southeast Wisconsin. Flexible visit scheduling, supervision of Personal Care Workers (PCWs), and care-plan development are core expectations.

The role requires an active WI RN license, at least 1 year of RN experience (home care preferred), CPR certification, and reliable transportation for home visits. Bilingual English/Spanish is a plus, not required.

Qualifications

• Active, unrestricted Wisconsin RN license (or compact license valid in WI).
• Minimum 1 year of RN experience; home care or long-term care preferred.
• Current CPR certification.
• Valid driver's license with reliable transportation and auto insurance.
• Ability to pass a caregiver background check.
• Strong assessment, documentation, and communication skills.
• Bilingual English/Spanish is a plus, not required.

Responsibilities

• Complete initial in-home nursing assessments for new clients using the Wisconsin PCST when required.
• Develop, review, and update individualized plans of care based on physician orders and client needs.
• Conduct supervisory home visits (at least every 60 days) to observe PCWs and verify care is delivered as planned.
• Train and orient PCWs on client tasks, transfers, infection control, and documentation.
• Reassess clients after hospitalizations, falls, or changes in condition and update the care plan promptly.
• Communicate with physicians, case managers, and families about client needs.
• Document visits, assessments, and care-plan changes accurately in the records system.
• Recognize and report suspected abuse, neglect, or safety concerns according to Wisconsin requirements.
• Support compliance with Wisconsin DHS 105.17 rules and Medicaid documentation standards.
