Village Caregiving is seeking a PRN Registered Nurse to join our home care team in Rhinelander, WI. You will visit patients in their homes, perform health assessments, document medical history, and develop personalized care plans for caregivers.

The role offers a flexible schedule and the opportunity to work independently while making a meaningful difference in the home care setting. Travel to client homes is required with support from the team.

Qualifications

• Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing, or Vocational Nursing.
• Current State RN License.
• CPR certification.
• Compassionate, friendly demeanor.
• Excellent communication skills.
• Driver’s license and reliable transportation.
• Ability to work a flexible schedule and travel.

Responsibilities

• Visit patients in their homes for one-on-one assessments.
• Complete initial and ongoing assessments to identify needs.
• Develop care plans for caregivers to follow.
• Educate patients and families on home health strategies and devices.
• Maintain clear communication with patients, families, and health professionals.
• Complete nurse assessment documents on a tablet and upload to CRM.
• Coordinate schedule with Director for upcoming assessments.

Village Caregiving -is looking for a PRN Registered Nurse to join our home care team! This role includes visiting clients to complete health assessments, document medical history, and develop personalized care plans for caregivers in the Rhinelander WI area . Must be willing to travel.

The RN may also assist with training new caregivers as needed. This position offers a flexible schedule and is a great opportunity for a nurse who enjoys working independently while making a difference in the home care setting.

Responsibilities

• Visit Patients in Their Homes for One-on-one Visits

• Complete Initial and Ongoing Assessments to Identify the Physical and Environmental Needs of Patients

• Assess Patients and Their Environment to Develop a Care Plan in Order for the Caregiver to Have a Blueprint to Know How to Safely and Competently Take Care of Our Patients

• Educate Patients and Their Families on Proper Home Health Care Strategies and Procedures, and Make Recommendations for Devices or Tools That Might Improve the Patient’s Quality of Life

• Provide Effective Communication to Patients/Clients, Their Family Members, Team Members, and Other Health Care Professionals

• Fill outNurse Assessment Documents on a Supplied Tablet the Same Day As Assessment. Upload Assessment to CRM.

• Keep Timely Communication with the Director on Schedule for Upcoming and Current Assessments.

Qualifications
