We are seeking a compassionate and skilledHome Health RNto provide care for our clients on apart-time, per-visit basisright here in Milwaukee. This isn’t your typical high-stress nursing job. Our visits are focused and efficient, typically lasting between 5 to 30 minutes.

You’ll have the autonomy to manage your schedule and visit route, with all documentation completed electronically at the point of care.

What You’ll Do

• Provide one-on-one skilled nursing care to clients in their homes.
• Perform patient assessments and develop care plans in collaboration with physicians and other team members.
• Complete timely and accurate electronic charting during your visits.
• Effectively manage your own schedule and travel route within the Milwaukee area.
• Serve as a trusted resource and communicator for patients and their families.

Physical Requirements

This role requires the ability to travel to client homes and perform essential nursing functions. You must be able to lift or move up to 50 pounds, assist with patient ambulation/repositioning, and stand, walk, stoop, and kneel as needed.
