Job summary

Village Caregiving is seeking a PRN Registered Nurse to join our home care team in the Eagle, River WI area. You will visit patients in their homes to complete health assessments and document medical history, then develop personalized care plans for caregivers.

The RN may assist with training new caregivers as needed. This role offers a flexible schedule and is ideal for a nurse who enjoys independent work while making a difference in home health care, with travel required.

Responsibilities

• Visit patients in their homes for one-on-one visits
• Complete initial and ongoing assessments to identify physical and environmental needs
• Develop care plans to guide caregivers on safely and competently caring for patients
• Educate patients and families on home health care strategies and device needs
• Provide clear communication with patients, families, and health care professionals
• Complete nurse assessment documentation on a tablet and upload to CRM
• Maintain timely communication with the director regarding schedules

Responsibilities

• Visit Patients in Their Homes for One-on-one Visits

• Complete Initial and Ongoing Assessments to Identify the Physical and Environmental Needs of Patients

• Assess Patients and Their Environment to Develop a Care Plan in Order for the Caregiver to Have a Blueprint to Know How to Safely and Competently Take Care of Our Patients

• Educate Patients and Their Families on Proper Home Health Care Strategies and Procedures, and Make Recommendations for Devices or Tools That Might Improve the Patient’s Quality of Life

• Provide Effective Communication to Patients/Clients, Their Family Members, Team Members, and Other Health Care Professionals

• Fill outNurse Assessment Documents on a Supplied Tablet the Same Day As Assessment. Upload Assessment to CRM.

• Keep Timely Communication with the Director on Schedule for Upcoming and Current Assessments.

Qualifications

• Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing, or Vocational Nursing

• Current State License As an RN

• Proof of Current CPR Certification

• Compassionate, Friendly Demeanor

• Excellent Communication Skills

• Current Driver’s License and Reliable Transportation

• Ability to Work a Flexible Schedule and Travel
