Job summary

iCare Health Care Services is seeking a Wisconsin-licensed Registered Nurse to conduct in-home client care needs assessments and provide clinical oversight of our caregiver staff, in compliance with Wisconsin DHS Chapter 105 regulations. This is a flexible, part-time/PRN position well suited to an RN seeking supplemental hours outside a primary clinical role.

Field-based role with visits across Milwaukee, Waukesha, and Ozaukee Counties, with flexible work-from-home options available.

Qualifications

• Active, unrestricted Wisconsin RN license.
• 1+ years of home health, home care, or community health nursing preferred.
• Familiarity with Wisconsin DHS Chapter 105 non-medical home care regulations a plus.
• Reliable transportation for in-home visits across the five-county service area.
• Strong documentation and communication skills.

Responsibilities

• Conduct in-home Client Care Needs Assessments for new clients and reassessments per DHS 105.17
• Develop and periodically update individualized plans of care
• Provide clinical supervision and competency oversight of caregiving staff
• Review and sign off on caregiver training documentation
• Serve as a clinical resource for care coordination questions from the intake and operations team
• Maintain accurate, compliant clinical documentation
