Job summary

Village Caregiving -Green Bay is seeking a PRN Registered Nurse to join our home care team in the Eagle River, WI area. You will visit clients to complete health assessments, document medical history, and develop personalized care plans for caregivers, with travel as needed.

The RN may assist training new caregivers. This flexible, independent role offers meaningful work in a home health setting and requires an active RN license, CPR, and reliable transportation.

Qualifications

• Graduate of an Accredited Diploma, Associate, or Bachelor’s School of Nursing.
• Current State License As an RN.
• Proof of Current CPR Certification.
• Compassionate, Friendly Demeanor.
• Excellent Communication Skills.
• Current Driver’s License and Reliable Transportation.
• Ability to Work a Flexible Schedule and Travel.

Responsibilities

• Visit Patients in Their Homes for One-on-one Visits.
• Complete Initial and Ongoing Assessments to Identify the Physical and Environmental Needs of Patients.
• Assess Patients and Their Environment to Develop a Care Plan for Caregivers.
• Educate Patients and Their Families on Home Health Care Strategies and Tools.
• Provide Clear Communication to Patients, Families, Team Members, and Other Health Professionals.
• Fill out Nurse Assessment Documents on a supplied Tablet and upload to CRM the same day.
• Keep timely communication with the Director on schedule for upcoming and current assessments.

• Provide Effective Communication to Patients/Clients, Their Family Members, Team Members, and Other Health Care Professionals

• Fill outNurse Assessment Documents on a Supplied Tablet the Same Day As Assessment. Upload Assessment to CRM.

• Keep Timely Communication with the Director on Schedule for Upcoming and Current Assessments.
