Responsibilities

• Provide high-quality, resident-centered nursing care.
• Assess, monitor, and document residents’ health conditions and progress.
• Administer medications and treatments as ordered.
• Identify and promptly communicate changes in resident condition.
• Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans.
• Provide education and support to residents and their families.
• Maintain accurate and timely documentation per facility policies and regulations.
• Promote a safe, comfortable, and therapeutic environment and supervise nursing assistants.

In this role, you will provide direct nursing care to residents while collaborating with physicians, rehabilitation professionals, and other members of the interdisciplinary team to ensure the highest standards of care within our long-term care and rehabilitation setting.
