Job summary

Jacksonville Health and Rehabilitation is expanding our Day Shift nursing team, offering multiple scheduling options for RNs to provide skilled care in a long-term care setting. You’ll work closely with physicians and the care team to support residents’ recovery, independence, and quality of life.

We offer diverse shift options (8-, 12-, 16-hour shifts and 7-on/7-off) and a range of benefits, including health, dental, vision, life insurance, and a 401(k) plan.

Qualifications

• Current RN license in good standing.
• Experience in long-term care, skilled nursing, or rehabilitation is a plus.
• Strong nursing assessment and clinical decision-making skills.
• Excellent communication and documentation skills.
• Ability to recognize and respond to changes in resident condition.
• Strong organization and time-management skills.
• Dependable and team-oriented approach to nursing.

Responsibilities

• Provide direct nursing care per resident care plans and physician orders.
• Perform nursing assessments and monitor residents for changes.
• Administer medications and treatments accurately.
• Document significant changes and notify the clinical team.
• Communicate with physicians and team members as needed.
• Complete accurate clinical documentation in a timely manner.
• Participate in care planning and interdisciplinary coordination.
• Assist with admissions, transfers, and discharges.
• Communicate effectively with residents, families, and staff.
• Supervise LPNs and CNAs and provide clinical direction as appropriate.
• Promote infection prevention, safety, and regulatory compliance.
• Maintain resident dignity, privacy, comfort, and independence.
• Provide leadership and clinical support to the nursing team.
• Follow facility policies and applicable regulations.

Shift Differential: $2.00 additional on base

What You’ll Do

As a Registered Nurse (RN), you’ll provide and coordinate skilled nursing care while working closely with physicians, CNAs, nursing leadership, and the interdisciplinary care team.

Your responsibilities will include:

• Provide direct nursing care in accordance with resident care plans and physician orders
• Perform nursing assessments and monitor residents for changes in condition
• Administer medications and treatments accurately and safely
• Recognize, document, and communicate significant changes in resident condition
• Notify physicians and other appropriate clinical team members as needed
• Complete accurate and timely clinical documentation
• Participate in resident care planning and interdisciplinary coordination
• Assist with admissions, transfers, and discharges
• Communicate effectively with residents, families, physicians, and other healthcare professionals
• Supervise and provide clinical direction to LPNs and CNAs as appropriate
• Promote infection prevention, resident safety, and regulatory compliance
• Maintain resident dignity, privacy, comfort, and independence
• Provide leadership and clinical support to the nursing team
• Follow facility policies and applicable federal and state regulations

What We’re Looking For

• Current Registered Nurse (RN) license in good standing
• Long-term care, skilled nursing, or rehabilitation experience is a plus
