Affirmed Home Care is expanding in the Hartford, CT region and is seeking Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide one-on-one in-home care. Flexible scheduling options include full-time, part-time, and per diem shifts with competitive hourly pay.

The role emphasizes compassionate, patient-centered care, adherence to care plans, and collaboration with a professional care team. Prior home care experience and CT licensure are preferred to join a supportive and growing

Formación

• Active RN or LPN license in CT is required.
• Minimum 1–2 years of nursing experience, preferably in home care or geriatric care.
• CPR/BLS certification required.
• Excellent communication and time management skills.
• Experience with Electronic Health Records (EHR) preferred.

Responsabilidades

• Conduct thorough patient assessments and implement individualized care plans.
• Administer medications and nursing procedures per physician orders.
• Monitor and document patient conditions, updating care teams promptly.
• Provide advanced nursing interventions including wound care, IV therapy, and catheter management for RNs.
• Educate patients and families on health maintenance, medications, and home safety.
• Maintain accurate documentation aligned with state, federal, and agency standards.

Key Responsibilities

Conduct thorough client assessments and implement individualized care plans.

Administer medications, treatments, and nursing procedures in accordance with physician orders.

Monitor, evaluate, and document client conditions, communicating updates promptly to care teams.

Deliver advanced nursing interventions, including wound care, IV therapy, and catheter management (RNs).

Educate clients and families about health maintenance, medication compliance, and home safety.

Maintain accurate, detailed documentation following regulatory and agency standards.

Ensure compliance with all state, federal, and company healthcare protocols.

Provide compassionate, patient-centered care that reflects Affi­rm­ed Home Care’s values of dignity and excellence.

Qualifications

Active RN or LPN License in the State of Connecticut (required)

Minimum 1–2 years of nursing experience (home care, geriatric, or related field preferred)

Strong assessment and clinical decision-making skills

Excellent communication, time management, and interpersonal abilities

CPR/BLS certification (required)

Reliable transportation and valid driver’s license

Experience with Electronic Health Records (EHR) preferred
