Job summary

Transitions Hospice is seeking an adaptable ECT Nurse (RN) to reinforce our interdisciplinary care team across Ottawa, IL and surrounding areas. You''ll provide high-quality nursing care in varied settings, delivering timely assessments, treatments, and compassionate support to patients and families.

This PRN role requires strong clinical judgment, excellent documentation, and willingness to travel locally.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Hospice/home health experience preferred.
• Flexibility to work across varied patient assignments.
• Strong assessment skills and clinical judgment under pressure.
• Excellent documentation habits and communication skills.
• Valid driver''s license and willingness to travel locally.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and necessary services are arranged and available for patients.
• Maintain and grow your clinical skills through continuing education and in-service participation.

What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
