Job summary

Transitions Hospice is seeking an experienced RN to join our Extended Care Team (ECT) as a nurse on an as-needed basis across Bloomington and surrounding areas. You’ll provide hands-on hospice care, collaborate with the interdisciplinary team, and respond quickly to patient needs.

You’ll bring active Illinois RN licensure, hospice or home health experience, strong assessment skills, and a willingness to travel locally.

Qualifications

• Active RN license in Illinois and accredited nursing education.
• Clinical hospice, home health, or related setting preferred.
• Flexibility to work across varied patient assignments and travel locally.
• Strong assessment, documentation, and communication skills.

Responsibilities

• Provide professional nursing care within scope of practice across visits.
• Conduct patient and family assessments and contribute to care plans.
• Complete visit types as needed to meet patient needs.
• Administer medications per physician orders and document outcomes.
• Identify nursing diagnoses and respond appropriately.
• Record timely clinical notes and monitor pain/symptom management.
• Communicate changes in patient condition to physicians and obtain orders.
• Coordinate with community partners to support transitions.
• Ensure equipment and services are arranged for patients.
• Maintain and grow clinical skills through continuing education.

What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:

• Delivering professional nursing care across the full nursing process within your scope of practice
• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing all visit types as needed to meet patient and family needs
• Administering medications and treatments per physician orders
• Identifying nursing diagnoses from health assessment data and responding accordingly
• Recording accurate, timely clinical notes including pain and symptom management outcomes
• Communicating changes in patient condition to physicians and obtaining orders as required
• Coordinating with community health partners to support seamless care transitions
• Ensuring equipment and necessary services are arranged and available for patients
• Maintaining and growing your clinical skills through continuing education and in-service participation

What You Bring

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred
• Flexibility and confidence working across varied patient assignments
• Strong assessment skills and sound clinical judgment under pressure
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally
