Job summary

Transitions Hospice in Pekin, IL is seeking an experienced ECT Nurse to join our interdisciplinary care team on a PRN as needed basis. You will provide on-demand nursing support across cases, delivering high-quality care with speed and clinical precision.

This role requires an active RN license, hospice/home health experience preferred, and willingness to travel locally to support patients and families. You’ll collaborate with physicians, care coordinators, and other team members to ensure

• Active RN license in Illinois.
• Graduate of an accredited nursing program.
• Hospice or home health experience preferred.
• Willingness to travel locally.
• Strong communication and documentation skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct thorough patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and necessary services are arranged and available for patients.
• Maintain and grow your clinical skills through continuing education and in-service participation.

What You'll Do

Requirements

What You Bring

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred
• Flexibility and confidence working across varied patient assignments
• Strong assessment skills and sound clinical judgment under pressure
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally
