Job summary

Transitions Hospice is seeking an experienced ECT RN to join our Extended Care Team. As an RN in this role, you will reinforce the interdisciplinary care team, assess and treat patients, and ensure timely communication of needs with speed and clinical precision.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred.
• Flexibility and willingness to travel locally.

Responsibilities

• Deliver nursing care across the full nursing process within your scope of practice.
• Conduct patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and necessary services are arranged and available for patients.
• Maintain and grow your clinical skills through continuing education and in-service participation.

What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:

• Conducting thorough patient and family assessments and contributing to individualized care plans
• Completing all visit types as needed to meet patient and family needs

• Recording accurate, timely clinical notes including pain and symptom management outcomes
